This area is used for visitors whom are in need of a mammogram and
require financial assistance. All
personal data gathered is optional. The information is
used to determine eligibility, degree of assistance needed, and for
subsequent contact.

information gathered is ONLY shared with the appropriate medical
provider or state agency after we receive your verbal or
written consent to forward this

Donations:
When you choose
to donate to the Breast Cancer Prevention Fund you must submit sensitive
credit card and personal information. The information collected is
transmitted via a secured server to our processing department.
Information required includes: name, address, email, telephone
number, bank card number and expiration date.
The information gathered
is used ONLY to process the donation and/or to contact the donating party
in order to complete the donation process.

always submitted directly to our offices via email where it is stored on a

processing purposes only.
